What is the Sacrament of the Altar, Lord's Supper, Holy Communion, Eucharist?
All of these are names for the same Sacrament. In this Sacrament, Christ gives us His true body and blood under the bread and wine for the forgiveness of our sins.
 
Where did this Sacrament come from?
Jesus Himself instituted it on the night on which He was betrayed (1Corinthians 11:23-24)
